Northeast Epidemiology Conference
October 15-17, 2014
A continuing education service of Penn State College of Health and Penn State Milton S. Hershey Medical Center in cooperation with the Philadelphia Department of Public Health and the Pennsylvania Department of Health.
Join us for the 2014 Northeast Epidemiology Conference on October 15 – 17, 2014, hosted by the Philadelphia Department of Public Health and the Pennsylvania Department of Health. State and Provincial epidemiologists from across the Northeast and Canada are invited to attend. The conference will feature a Wednesday evening welcome reception, Thursday night dinner with a special guest speaker, plenary sessions and breakout sessions in Infectious Diseases, Cross-Cutting, Chronic Diseases, and Occupational Health/Environmental Health. Continuing Education credits will be offered in conjunction with Penn State College of Medicine.

The conference will be held at the Sheraton Philadelphia Society Hill, 1 Dock St. (2nd & Walnut Sts.), Philadelphia, PA at the rate of $163/night. The room block cutoff at the Sheraton Philadelphia Society Hill is September 15 at 5:00 pm. Please make your reservation early as rooms within the discounted room block are limited.

This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Penn State College of Medicine and the Philadelphia Department of Public Health and the Pennsylvania Department of Health. Penn State College of Medicine is accredited by the ACCME to provide continuing medical education for physicians.
Nurses will receive a certificate from Penn State College of Medicine verifying their participation in approved continuing medical education. Continuing education activities which are approved by accredited institutions of higher education and that are relevant to patient care or professional nursing meet the requirements of mandatory continuing education for registered nurses in Pennsylvania, as required by the Pennsylvania State Board of Nursing. The Board of Nursing recommends that nurses retain their certificates for a period of at least five years. Penn State has approved this program for Continuing Education Units. The CEU is a nationally recognized unit of measure used to accumulate a standardized permanent record of an individual's participation in continuing education programs conducted under responsible sponsorship, capable direction, and qualified instruction. It is based on a standard of one unit per ten hours of instruction. Upon completion, participants will receive a certificate of CEUs earned.
Please register for the conference by Sept. 22. 2014. All conference attendees and speakers are responsible for making their own hotel accomadatons. the hotel room block deadline is September 15, 2014.

From Philadelphia International Airport
Take Interstate 95 North to Exit 20 (Columbus Boulevard). Turn left at the traffic light onto Columbus. Turn left turn at the sixth light, for Dock Street. Turn right at the stop sign. The hotel is on the right-hand side.
Take Ben Franklin Bridge from Camden, stay in the right lane. Take the first right off of bridge to Sixth Street then follow Sixth Street to Market Street and turn left. Follow Market to Second Street and turn right. Follow Second Street to Dock Street and the hotel is on the left.
Take Interstate 95 South to Exit 20 (Columbus Boulevard). At the bottom of the ramp, turn left onto Columbus Boulevard. Follow for 3 traffic lights to Dock Street and turn left. Continue to a dead end and turn right. The hotel will be on the right.
Take Pennsylvania Turnpike to 76 East (Exit 326 - Valley Forge). Follow 76 East to 676 East (Exit 344 - Central Philadelphia). Continue on 676 East to Interstate 95 South. Take I-95 South to Penn's Landing (Exit 20). At the bottom of the ramp, turn left onto Columbus Boulevard. Follow to Dock Street and turn left. Continue to the dead end and turn right. The hotel is on the right-hand side.
Discounted valet parking at the Sheraton Society Hill is available to all conference attendees at the price of $41.04 per day. However, there is an alternative parking facility called Ritz Parking located on 2nd Street between Chestnut Street and Walnut Street. This facility charges $19.00 per day, and is also available at hourly rates.

From Greyhound Bus Terminal (1001 Filbert Street)
Walk west to 11th Street. Take a left onto 11th Street. Enter the Market-Frankford line (SEPTA subway service) stop at 11th Street and Market Street. Take the Market-Frankford line east toward Frankford Transportation Center. Exit at the 2nd Street stop. Walk south along 2nd Street (toward Chestnut Street) to Dock Street. Turn right onto Dock Street. The hotel will be on the right-hand side.

To complete the Speaker Interest Survey, you will be asked to submit the following:
1) Presenting author name and contact information
2) Presentation title
3) Brief description of the presentation/topic (It is not necessary to provide a full abstract) and preferred format (oral presentation or poster)
4) Three learning objectives for your talk
5) A brief biography of the presenting author
6) Subject area keywords (infectious disease, environmental health, chronic disease, or late breaker).
You may submit more than one presentation topic if you wish. To do so, please complete a separate Speaker Interest Survey for each proposed presentation.
Please note that all conference speakers are also asked to register for the conference by Sept. 22, 2014. In addition, all conference attendees and speakers are responsible for making their own hotel accommodations; the hotel room block deadline is Sept. 15, 2014.
