Requirements for Graduating/Terminating Housestaff
As you are preparing to leave Penn State Hershey,
1. Your current bank account MUST REMAIN OPEN until you receive your last paycheck.
Your FINAL paycheck will be direct deposited into your current bank account. The paycheck stub will be mailed to the address you provide on the Resident Change of Address Form.
2. Complete the Resident Change of Address Form at https://www.new-innov.com/Login/Login.aspx
Completion of this form will ensure that you receive your last paycheck stub and W2 statement in January 2014.
3. Print both the NPI Letter and the Malpractice Face Sheet.
Your NPI Letter and Malpractice Face Sheet are attached to your file in New Innovations. This information is REQUIRED when applying for residency, fellowship or clinical privileges in the future. PLEASE PRINT this information and keep it in a safe place. Instructions for printing.
4. COBRA:
If you do not have healthcare benefits secured for July 1, 2013, you may elect to purchase COBRA health insurance. Information provided by Human Resources will be sent to your home mid-June. If you have questions regarding COBRA, please contact Penn State Hershey Benefits at 717-531-6089.
