Scheduling Appointments
You may schedule an appointment by calling 800-243-1455.
It is important for you to understand your treatment plan, so we encourage questions. Before you come in for your appointment, we suggest that you take a moment to write down any questions you want to ask the physician.
When you arrive at the office, please tell the staff if you have changed your address, telephone number, primary doctor, insurance coverage or other important information.
Before Your First Appointment
When you schedule your first appointment, our staff may ask about the nature of your problem and your physician's arrangements for your care. This helps us provide the most efficient plan for your care.
We need all of your medical records related to your condition at the time of your first visit. This includes any X-rays or previous surgery records.
If you need to obtain X-rays or records from a doctor other than the physician who referred you, please have these sent to us before your first appointment.
It is also very important for our medical staff to know exactly what medications you take and if you have any known drug allergies. Please bring this information with you for your office appointment.
Delays
Due to the nature of our practice, we may have to delay your appointment time while your physician deals with an emergency or a patient with a complicated problem. We know your wait is inconvenient, and we appreciate your understanding.
If an emergency causes us to cancel your appointment, we apologize for any inconvenience. We will reschedule your appointment as soon as possible.
Canceling
We ask for at least 24 hours notice when you need to cancel or change your appointment. This gives us the opportunity to give an appointment to a patient with an urgent need. Please let us know as soon as possible if you are unable to keep your appointment.
